If You Cannot Use the Online Bundle
If you cannot register online, you can mail a completed Statement of Live Birth form to the Office of the Registrar General. The form is available at hospitals and from ServiceOntario. If you register by mail, you will need to apply separately for a birth certificate, SIN, and Canada Child Benefit.
If your child is older than 12 months and the birth has not been registered yet, the process is longer and costs $50. You cannot use the online Newborn Bundle. Contact ServiceOntario at 1-800-461-2156 for instructions.
OHIP for Your Newborn
OHIP registration for your baby is handled separately from the birth registration. If your baby was born in an Ontario hospital with birthing facilities or with a registered midwife, the hospital or midwife will give you an Ontario Health Coverage Infant Registration form. Complete it and they submit it on your behalf. Your baby's health card will arrive by mail within about 8 weeks.
If your baby was born at home without a midwife, or in a hospital without birthing facilities, you need to apply for OHIP in person at a ServiceOntario centre.
